Objective: To analyze the difference of blood glucose level in intervention of acupuncture combine with Metformin 500mg tablet and intervention of just Metformin 500mg tablet in Diabetes Mellitus type 2. Methods: A randomised controlled trial with six weeks follow up were done. This research was randomized pre-post test. Total of 85 people patients with diabetes mellitus type 2 come as acupuncture patients in “Rungkut” private clinic Surabaya, Indonesia, from January 2013 to January 2014. After screening 73 subjects who had met the inclusion criteria were invited to participate in the study. 13 subjects dropped out because of discontinued the treatment, the reasons were too busy to participate due to work schedule conflict, family issues, feeling unwell, moved to another town. The total sample were 60 patients divide into 2 group, group control and treatment group, each consists of 30 patients. The control group is given Metformin 500mg tablet 2 times per day for 6 weeks. The treatment group is given Metformin 500mg tablet 2 times daily and acupuncture three times a week for 6 weeks, using Huanqiu steril acupuncture needles, specification 0.25 x 40 mm (1.5 cun). The acupuncture point principle use Zusanli (ST36) Location 1 finger breadth lateral to lower end of tibia tuberosity. Puncture – 1 cun perpendicularly and Taibai (Sp 3) Location - Posterior and inferior to head of 1st metatarsal bone at border of two colon of skin. Both group activities, exercise and diet as usual with no change. The age’s data analyzed using frequencies test; the gender’s data using fisher’s exact test; the education’s data using crosstab test; the duration of diabetes’ data using descriptive test; the corellation age and Fasting Plasma Glucose (FPG) ... ... middle of paper ... ...rol level is -14.17 mg/dl, p=0.001. According to the standard TCM Diabetes effectiveness in the treatment group: healing was 10%, which is 50% efficacious, inefficacious is 40%, whereas the control group: 6.67% healing is, 33.33% efficacious, inefficacious is 60%. The overall success rate in the treatment group was 60% and the control group was 40%. Standard efficacy of TCM, the comparison between treatment and control groups showed both were equally effective, meaning that symptoms improved after treatment, but in this case it is more effective in the treatment group 20% greater than the control group. Conclusion: The use of acupunture points combine medicine in this research use Metformin 500mg tablet twice daily, there is significant difference in the therapeutic magnitude of decrease in total cholestrol level p=0.001 though for magnitude of decrease in Fasting
For this evaluation of a clinical practice guideline I chose to evaluate a guideline that was published in 2012 and that addressed the use of oral medication in the treatment of Type 2 Diabetes (Appendix B). People who have received a diagnosis of Type 2 Diabetes Mellitus (T2DM) are facing a long term treatment plan. Diabetes mellitus is a chronic metabolic disorder that results from a problem with insulin in the body. T2DM is growing in prevalence and is a cause for concern. There are several co-morbidities that can be linked to poorly controlled blood sugar levels such as cardiovascular disease, peripheral vascular disease, renal failure, and a decrease in eyesight. If blood sugar levels can be maintained at a normal to near normal limits the patient has a decreased risk of experiencing these comorbidities. Because of this it is important for the healthcare practitioner to evaluate all treatment methods available and the evidence that supports the efficacy of recommending this treatment to our patients. I used the AGREE II tool in evaluation of this guideline (Appendix A).

Since the first report on the success of acupuncture anesthesia in 1970s, much attention has been attracted to the effectiveness of acupuncture therapy worldwide. Analgesia is one of the most important effects of acupuncture. Generally, mechanisms of acupuncture analgesia contain mechanisms of acupuncture anesthesia, but the latter does not represent the entirety of acupuncture analgesia. This is because acupuncture not only treats acute or transient pain, but also chronic or persistent pain resulted from inflammations or other causes. Clinically, the pain usually occurs prior to acupuncture, either needling or moxibustion can be used for treatment. For acupuncture anesthesia, an induction period of acupuncture is required prior to the surgery and only needling or acupressure may be applied. In addition, the surgical pain pertains to the category of acute pain. However, most modern studies on mechanisms of acupuncture analgesia are conducted focusing on the mechanisms of acupuncture anesthesia. In the following discussion, we will first outlook those studies on acupuncture anesthesia, then provide a complementary explanation on mechanisms of clinical acupuncture analgesia, and finally analyze their implications in improving results of clinical analgesia. Primarily, mechanisms of acupuncture anesthesia or analgesia include two closely associated aspects: neural and humoral mechanisms [27].

Glucose is a sugar that plays a big part in a human’s health and well-being. This sugar is a major source of energy for the body’s brain and cells. The Cells that receive energy from glucose help in the building of the body’s muscle and tissue. Although glucose may be important to the body too much of this sugar can cause a chronic condition called Diabetes. Diabetes, also known as Diabetes mellitus, is a chronic condition that is caused by too much sugar in the blood. This condition can affect all age groups. In fact, in 2010 a survey was taken by the National Diabetes Information Clearinghouse, on the number of newly diagnosed diabetes. Out of 1,907,000 people: 24.38% were ages 20-44, 55.17% were ages 45-64, and 20.45% were ages 65 and greater. Diabetes is a very serious condition, and it can be deadly if left untreated. This paper will help better educate the reader on the signs and symptoms, the testing process, and the management of diabetes.

The NDSS also works to improve the self-management of diabetes. Physiotherapists also have a role in the management of diabetes through implementing exercise programs. An evidence-based approach was used to create the proposed exercise program that aimed to manage the high blood glucose levels associated with insulin resistance in type 2 diabetes (Sigal et al., 2007). It utilized both aerobic and resistance training in accordance with current knowledge of the most effective dosages for the population group. Physiotherapists must work alongside other healthcare professionals such as dietitians and general practitioners to provide optimal support and management for patients with diabetes.

From then on, physicians began to gain a better understanding about diabetes. Centuries later, people known as "water tasters" diagnosed diabetes by tasting the urine of people suspected to have it. If urine tasted sweet, diabetes was diagnosed. To acknowledge this feature, in 1675 the word "mellitus," meaning honey, was added to the name "diabetes," meaning siphon. It wasn't until the 1800s that scientists developed chemical tests to detect the presence of sugar in the urine”.
Acupuncture doesn’t allow people to get ahead of themselves like Victor did; the way of thinking brought about is that of a natural sense. The techniques of the alternative medicine allow people to remember how magnificent and sacred the human body is and realize that it cannot be created through lab work or perfected through medicine. With Western medicine, patients can take up to four or five medications before finding one that suits their needs and most can have many life threatening side effects. With acupuncture, it is one procedure and one attempt to heal a patient with NO major side effects. Acupuncture is a unique ancient art that is backed by thousands of years of Chinese thought and research and has been proven affective for many people and their illnesses.
