East Side Community Assessment and Windshield Survey
According to Allender, Rector, and Warner (2014), public health is a combination of both an art and a science (2014). The mission of public health nursing is to promote health, prevent disease and ultimately prolong life (Allender et al., 2014). In order for this to occur an assessment must take place. An aggregate or community assessment begins with a collection of data. This includes: the community’s health needs, risks, environmental conditions, financial resources through local census data, and a windshield survey (Allender et al., 2014). Through public health nursing, communities can collectively come together to help promote an overall better health standing.
My group’s aggregate is in East Side Community of Buffalo, NY. For this assignment, a windshield survey of East Side Community was completed. The windshield survey is an assessment of the community through my personal observations. This survey will include an assessment of four categories. These consist of the physical aspects, economical standings, services provided, and social aspects of the community. In addition to the windshield survey, local census data was collected. Below are east side community’s results and local census data findings.
East Side Community: Population Characteristics
East side community is in the city of Buffalo, NY. Buffalo, New York is the second largest city in the state and is a part of Erie County (Erie county overview, 2012). The Robert Wood Johnson Foundation (RWJF), a philanthropy organization, has been involved in health matters facing Americans for over 40 years with focus to child and family wellbeing, health coverage, health leadership and workforce, health systems improvements and healthy weight. Since its inception in 1972, RWJF support has grown and is now the largest philanthropy body devoted exclusively to health in the nation (RWJF, 2015). In this paper, the author discusses the RWJF Committee initiatives and their effect on nursing education, nursing practice and nursing workforce development, the role of action based coalition and expound on the state of Oklahoma Campaign team report.
The educator role of the APHN and NP includes health education within a nursing framework and professional nurse educator roles. The APHN analyzes groups at risk within a community and implements health education interventions. The APHN and NP boost wellness and contribute to conserving and advocating health by teaching the importance of a healthy lifestyle, stress management, physical exercise, and good nutrition. They educate about disease processes and the importance of following treatment systems. In addition, they provide guidance and educate clients on the use of birth control methods, diet, medications, and other therapeutic procedures. At one end are the residential communities of East Setauket/Stony Brook – split-level ranches with multiple cars in the driveway, lawns that are diligently watered (despite the ongoing drought here) and mowed, the studied quiet of Long Island suburbia. At the other end, the trail literally empties out into a lot in Port Jefferson Station where homeless individuals curl up under trees for shade and sleep in summer and food pantries routinely draw large crowds on the weekend. The American Nurses Association (ANA) developed a foundation for which all nurses are expected to perform their basic duties in order to meet the needs of the society we serve. The ANA “has long been instrumental in the development of three foundational documents for professional nursing; its code of ethics, its scope and standards of practice, ands statement of social policy.” (ANA, 2010, p. 87) The ANA defined nursing as “the protection, promotion, and optimization of health and abilities, prevention of illness and injury, alleviation of suffering through the diagnosis and treatment of human response, and advocacy in the care of individuals, families, communities, and populations” and used to create the scope and standards of nursing practice. Healthcare is a continuous emerging industry across the world. With our ever changing life styles and the increased levels of pollution across the world more and more people are suffering from various health issues. Nursing is an extremely diverse profession and among the highest educated with several levels ranging from a licensed practical nurse (LPN) to a registered nurse (RN) on up to a Doctorate in Nursing. Diane Viens (2003) states that ‘The NP is a critical member of the workforce to assume the leadership roles within practice, education, research, health systems, and health policy’.
There are a variety of definitions that organizations create to aid in a better understanding of the importance nursing holds in providing safe, patient centered health care to the community. For example, The International Council of Nurses states that “nursing encompasses autonomous and collaborative care of individuals of all ages, families, groups, and communities sick or well, and in all settings. Nursing includes the promotion of health; prevention of illness; and the care of ill, disabled, and dying people. Advocacy, promotion of a safe environment, research, participation in shaping health policy and in patient and health systems management, and education are also key nursing roles” (Potter & Perry, 2016, p.3).
Many communities, regions, and states are working together to create the culture of health by redefining the meaning of getting heathy and staying healthy. These communities, regions and states are addressing the social, physical, economic environment that people live in. The Americans can overcome the multitude of disparities and creates equalities with the help that directs resources, research, compassion and community involvements (Maughan, 2014). Health disparities are not just a health issue, it is a combination of financial issues, racial issues, education issues and others. Nursing leaders can address these health disparities differently. For example,
A community can be defined as a group of people, who live, learn, work and play in an environment at a given time. (Yiu, 2012, p.213) There are many factors that may influence the community’s development and health status. These can include resources available, accessibility, transportation, safety, community needs etc… These influences may combine together to form community strengths and as well as community challenges or weaknesses. As a community health care nurse, it is significant for us to assess and identify these strengths and challenges within the community in order for us to intervene and provide the appropriate needed health care services for the community members. This individual scholarly paper will explore and focus on one challenge issue identified from our group community assessment.

Our group definition of health showed that health is broad and consists of objective and subjective parts. In contrast, the king’s interaction model (theory of goal attainment), the purpose of nursing is to help people maintain, or restore health by mutually goal setting. Her view towards the concept of health are easy to understand, but difficult to realize because the role of nurses is based on the health needs of patient, family, community and population. The other needs of patient, family and community will be resolve by nurses giving patients referrals or follow-up for care to other professionals. During one of my undergraduate courses, “Community and Population Health”, I completed a paper on my community and access to healthcare. During the research and community outreach performed to meet the goals of the paper, my eyes were opened to the plight of small communities in regards to access to quality healthcare. This plight has become my passion, and has formed the basis for my vision of the Family Nurse Practitioner role.
New York City’s population is a little over 8.3 million people. 8.3 million people are spread out among five boroughs and each have their own set routine. Each one of those 8.3 million see New York in a different way becuase “You start building your private New York the first time you lay eyes on it” (“City Limits” 4). Some people are like Colson Whitehead who “was born here and thus ruined for anywhere else” (“City Limits” 3). Others may have “moved here a couple years ago for a job. Maybe [they] came here for school” (“City Limits” 3). Different reasons have brought these people together. They are grouped as New Yorkers, but many times, living in New York is their only bond. With on going changes and never ending commotion, it is hard to define New York and its inhabitants in simple terms.
Resource utilization is important in nursing care delivery. ANA standards of nursing providing some stratergeis regarding resource utilization. The responsibility of the public health nurse is to assess the availability of resource and individual health needs to complete the healthcare tasks. The public health nurse considering certain factors that belongs to health care cost , safety , effectiveness and impact on nursing care in the planning and nursing care delivery. Also public health nurses are evaluating as such factors are bringing the expected outcome and its effectiveness. By the help of public health nurse the client and family members can identify the health services appropriate for health care needs.
Nurses are at the forefront of the healthcare system and therefore must actively engage in policies that impact the healthcare system. DNP graduates can initiate the process of change by pointing out potential problems within the healthcare delivery system and spearheading legislation through lobbyists and nursing organizations. Nurses should be the middle-man between patients and policy makers. Patient advocacy has been part of nursing for a long time. Zaccagnini and White (2014) highlighted how Florence Nightingale advocated for the British soldiers’ poor living conditions during the Crimean war once she recognized the connection between policy makers and public health. Local, national and global nursing organizations provide ways for nurses to voice issues and concerns that affect public health welfare. In the United States of America, the Center for Disease Control (CDC) and the National Council of State Board of Nursing(NCSBN) can be used as a platform to raise awareness and concerns for public health and
Community engagement is the active participation of local residents and community groups in the decisions that affect their lives (Herefordshire Council, 2013). Therefore, community engagement should be about engaging in open communication to ensure the council understands the needs of the local community.
