By giving nurses a sense of identity, nursing theory can help patients, managers and other healthcare professionals to recognize the unique contribution that nurses make to the healthcare service (Draper 1990).Hildegard Peplau, one of the role model of nursing theorist, established a theory of interpersonal relationships in nursing. Her reason is the goal of the nurse-client relationship is to give a quality nursing care moving forward to health promotion and maintenance. The purpose of this paper is to conduct a basic assessment of her theory.
Theory Vs conceptual model
It is important to first define theory and conceptual model in order to classified Pepau’s theory. Karlowicz K. A. states : “Theories are composed of concepts, definitions, models, propositions and assumptions that, when organized, offer a systematic view of a phenomenon (an observable fact or event) that is descriptive, explanatory & predictive, and directive in nature. Pepau’s theory of interpersonal relation of has a significance worldwide influence in nursing profession. She intensifies apprehension of nature and matter of interpersonal relation between all nurse and patient. This assignment helps me understand that partnership between my client and me. I try to be more active listener, validate my client inferences and check for accuracy. The client often enjoys that and outcome is beneficial for both. Pepau (1992) regard the human being as a client who merits “all of the humane considerations; respect, dignity, privacy, confidentiality, and ethical care”. Hence the goal of a nurse is” to assist patients to become aware of and to solve their problems that interfere with constructive living”.
Positive relationships with both nurse and client is rewarding to both sides; however, there are essential elements needed to achieve a good nurse-client relationship. Multiple studies were analyzed by Hagerty and Patusky in “Reconceptualizing the Nurse-Patient Relationship” to determine the elements of linearity, trust, time, and role expectations within the nurse-client relationship. Linearity allows the relationship to act in “phases”, which provides efficient and quality care. By doing so, a nurse must establish trust, work with the client while progress continues, and when client is free to go home, the relationship comes to an end. During the orientation phase, trust must be established; as this provides the client comfort and confidence to provide valid information, and then receiving proper care. Effective use of time is vital to the relationship, though many nurses believe they do not have enough time to provide care due to the current health care pressures and time limitations. A study by Forchuck, then analyzed by Hagerty and Patusky, was conducted between 10 nurse-client relationships; Forchuck reports that 7 out of the 10 established a “working relationship” between nurse and client within 2.5 and 6 weeks. Regardless of time, many nurses are introduced to a client with the presumptions that the client wants to receive care and create a relationship, though the nurse expects nothing in return. “Heifner (1993) found that nurses identified vulnerability of patients as an essential foundation of nurses’ abilities to ‘connect’ with patients” (Hagerty and Patusky, 2003, 146). Despite nurses building a relationship upon vulnerability, Hewison discovers that control can actually create “barriers to open and meaningful communication and exchange” (2003). This paper explores the personal philosophy I have as a nursing student and what I intend to convey throughout my nursing career. The Theory of Interpersonal Relations is a process that starts with the roles of the nurse, and those roles began with the nurse as a stranger, teacher, resource person, counselor, surrogate and leader (Nursing Theories, 2012). In “Philosophy and Theory of Transpersonal Caring” by Jean Watson she believes that nurses hold an impact that is strongly based on the caring relationship between a nurse and a patient. The theory focuses on how nurses care for their patients, and how that care may help promote, prevent, or restore a patient’s health. Watson states “caring is essential in nursing, and requires the nurse to have a deep connection to the spirit within the self and to the spirit within the patient” (Lachman, 2012, p.112).
Nursing and Qualities That I Possess to become a Good Nurse Nursing is the act of safely caring, protecting and improving our clients’/patients’ health and ability without causing any further harm or disability to them. Our primary goal is to restore and maintain good health physically, spiritually and psychologically. It is a science, such that one has to apply the nursing knowledge and technical aspects of practice. However, it is important to incorporate the act of patient-centered care, which is defined according to QSEN/NOF, as holistic care that recognizes the patient as the source of control and full-partner in providing compassionate and coordinated care with respect for the patient’s desire, values, needs and preferences. It has to incorporate with the nurse’s personality, i.e., certain qualities that the individual possess.
When a nurse is providing patient care, he/she creates a safe environment for the patient and enables the choice to establish a relationship on a human to human interaction or on a transpersonal level. The patient will be acknowledged as a person with the wholeness of their soul despite their illness or number on the bed.
